I think my friend should pay, she thinks we should split it?

Driving a friend 4 hours away for an operation (cosmetic) and will use 2 full tanks of fuel in my car, cost of a hotel, food while I'm there and I'm using a days annual leave.
She wants to split all costs down the middle, whereas I think I'm using my holiday entitlement and 2 days of my time (which I am more than happy to give up) but don't think I should be paying for fuel, or for the hotel? I've said I'll pay for all my own food.
Her logic is we're both going so why should only she pay, she's already paying lots for surgery.
My logic is I wouldn't be going if it wasn't to do her a favour so don't see it fair to split costs.
Whats the right thing to do here? Don't want to be mean but trying to be fair...

You’ve given up your time and she wants you to pay and is being nasty because you won’t.

If I was getting surgery and going under GA practially in a different country (I’m assuming that you’re driving from wales to london?), and had to get a friend to drive me down, look after me immediately after then drive us back - i wouldn’t DREAM of asking them to pay! Let’s be honest it’s a massive inconvenience and there is 0 benefit to the driver.

London is a fabulous city but you can’t enjoy it - you’ll have to stay near the hospital/hotel and care for her. The only alone time you’ll really have is whilst she’s having the procedure...

I had minor surgery under GA in March, my friend collected me and we took an uber home...it didn’t even cross my mind to split the taxi cost!

Bare in mind that the journey back won’t be straightforward as she’ll be recovering from the surgery. What happens if she begins to feel dizzy or nauseous, or feels uncomfortable sitting upright and gets a sharp pain etc during the drive back. She might need help walking or going to the bathroom if she feels a bit dizzy. A long motorway trip just doesn’t seem sensible. I would just tell her that she should book the hotel for longer then make her way back via train when she feels ready.

She should pay the extra to pay for herself to stay at private hospital for at least 48 hours but really 72 hours after surgery
