Hi 👋

A thread for anyone who wants to discuss their Create Fertility experiences.

I'm 38, currently in a 3 cycle Natural Modfied ICSI package and nearing transfer stage.

@jolly86 I think it depends on when the egg was fertilised. What time did you have collection? I think there will be some variation of 'normal'.
I do understand you feeling less hopeful, I was the same last year when we transferred and neither embryo was optimal. The embryologist called me the day after and we had a good chat.
I know you'll be feeling disappointed and I'm sorry ❤️

I hope that your other 2 continue to develop well and you get good news on Monday xx

@VenusStarr I had another chat with embryology and actually I asked why they don't transfer both the grade 2 and grade 3, 7 cells one? As given the percentages of each becoming a blastocyst, then become a pregnancy, it just makes sense that I get both of them. Rather than possibility of ending at day 5 with a low grade embryo not worth freezing? Would you agree?

@jolly86 I think that sounds logical but of course Embryologist would be the one to advise, I know our Dr was reluctant to have me do a double transfer until 2 transfers have failed so its more of a clinical decision I think. We have a 12 cell grade 2 day 3 in the freezer and I had the same thoughts about whether it had progresses too quick, be curious to know what advice you get, we won't know how it does until we thaw it (I've kind of written it off in my head for those reasons).

@VenusStarr I'm ok thanks, had the Hysteroscopy Consultation and it does appear to be a polyp, so glad it got picked up as it can cause issues for implantation. So I'm having it sorted in the new year hopefully in time to start the FET end of January. It is suspected that the stimulation had caused it, apparently happens in 1 in 15 cases and sods law I was one of those 15. Not surprised though having done 3 collections in less than 4 months. Regarding ICSI...I would certainly be an advocate for it, I know its more money but if it gives the process a helping hand then its worth it in my mind. Like I said my OH's SA was really good, despite slightly lower morphology (2%) his numbers and motility were excellent so they weren't concerned by the morphology as that was 2% of a high number, but for whatever reason we had failed fertilisation in our 1st NHS cycle-was it down to egg quality at the time (I was max stims, and I hadn't made the diet or supplement changes) I won't know, but in all honesty I wouldn't want to take the chance especially as our fertilisation rate has improved massively since having ICSI/ making diet & supplement changes. Its hard to attribute exactly what's improved it but its rather not strip out ICSI - not a gamble I want to take when going through stimulation then egg collection

@Sunflower360 I know but double transfer on day 5 isn't the same as on day 3 right? I feel clinics are too risk averse. Given a day 3 embryo will only become a blastocyt with 30 -40 percent by day 5, this risk seems very very low?

@jolly86 definitely agree on them being risk averse, I was hoping to do a double transfer of this 1st one doesn't work seem as we've also had a natural chemical pregnancy but it got shot down pretty quick. I think it's worth asking and hopefully they'll do it. Are you having the transfer later today? Hope it goes ok.

@Sunflower360 yes I am having this afternoon, I will definitely be pushing for a double transfer (they said we will have a chat so we will arrive a bit before), as I am not under 35, plus they are not grade 1 top quality ones either! I think the risks are lower and I don't want to be in a position in two days to freeze a possibly low grade embryo.

@jolly86 its definitely worth exploring a double transfer, I don't know what the odds are for a double day 3 but they say a double day 5 only slightly increases the chances. But I know they generally would only do 2 average grades rather than a good and poor one. It's a bit of a minefield. How would you feel if both took?

@Doxy21 are you day 2 today? I don't remember a day 2 call, unless they've changed? But would think they'll check them tomorrow and see how they're doing and call you. I vaguely remember my day 1 call they provisionally booked my day 3 transfer but then got a call early to say all fine, don't come til day 5. But if you're worried, give them a call to check.

@Sunflower360 yes, I'm leaning towards trying it as we had 4 eggs not fertilise the first time, so I think I want to try. We have budgeted for it.
Dh is coming down with something...typical. I was really ill a few weeks ago and he swerved that. Think its just a cold.

@Doxy21 they don't call on day 2 though. I didn't get an update either. They call on the morning of day 3. Tbh my understanding is there is always something to transfer on day 3, as most embryos reach that day but may be low quality. The drop off is 3 to 5, which we won't see if having a day 3 transfer.

Thanks all, yes fresh transfer but I’ve been given no instructions for day 3 eg time, what to do on prep etc? I thought they would call today, my husband also wanted to ask about transferring 2 - is that a decision you can make on the day?

@Doxy21 on egg collection they provisionally book you for egg transfer day 3. There aren't specific instructions, and transfers are in the afternoon so they don't need to confirm until morning of day 3. I would think you prepare yourself for a transfer tomorrow. They called me before 8 am this morning. Did they say anything about sunday transfers? I am sure they do them right?

@Doxy21 yes I missed the call several times they called back but there's no way for us to call back so yeah make sure you wake and keep your phone from 7.30 am.

I'm so glad they saw your polyp. I've read they can affect implantation @Sunflower360 that's interesting about why you might have developed it... another thing to watch out for! Glad you've got a plan x

Thanks @VenusStarr Thankfully the Dr who's done all my egg collections was in the scan where he noticed it as sometimes its just the nurses who the do the scans (maybe they would have picked it up too but I'm not sure as they don't really look out for things like that normally I don't think bar loning thickness and follicle sizes. But yes it is something else to watch out for so I wanted to share in case anyone else found it interesting or point to investigate further

@Doxy21 I asked for a call back to talk with embryologist before going and they called me, then decided I would speak to dr before transfer. The dr told me of risk of multiple pregnancy but she was aware this is a very small risk hence didn’t have to push hard and they agreed to do the double.

@Doxy21 not really as it's me who brought this up not them, they didn't encourage but didn't discourage either. In my case I didn't see the point of leaving one out, and the probability of both implanting is very very low so not much risk.

Congrats on being pupo @jolly86 🥰 are you testing at home or will you do a beta blood test? I'm glad they let you do a double transfer.

Ah ok. Did they tell you the grade of the embryos @jolly86 ? (I know they are graded differently at day 3 to day 5). They haven’t told me any of that yet.
