Low temperature and poor ventilation. Insulate and open a window.

Maybe cold spots due to gaps in insulation or cold bridging (where building elements touch walls and draw heat away). Say, plaster on the hard or maybe an external wall up against the internal one, joist, etc). You also need to kill the mould with a mould killer to stop it coming back, but the basic problem is condensation and that only happens when surface is cold.

In some buildings over insulation can be a problem, too, but this gets a bit complicated - for detailed advice I'd ask a specialist!
