Anything from the obvious naming of the "good guy" mr white (or Blanc, or snow etc) to more seemingly obscure indicators of character or purpose - inspector morse - yes he's a code breaker but he's also something of an enigma, a code himself. Sometimes it's intended as a clue to upcoming events eg a murderer fixated with ideologically "killing the King" may target a character named Rex or something similar. Or God/goddess names...

A dream sequence would be a total cop out! I'd hope any decent writer outgrows such ideas early on.

The death notification to Julia's mother is just the writer being ignorant of how it would actually be done or else taking dramatic licence. It's what eventually really put me off shows like casualty. It's annoying enough to see back to front X-rays or wrongly set drips but to get people thinking that medical staff would be so insensitive as to tell people such awful news basically in public boils my piss!
