No, I don't think Ros things Nick cut off her arm (although you can see why she might suspect that given the whole Ifield/saw incident in ep 1). She means he gave consent for the amputation whilst she was unconscious. So she blames him for it.

Yes, consent not cutting it off himself. I think questions would be asked if you turned up in A&E missing a limb.
