I did ask this at one stage - but no - you've to send it directly to them - in my last post (just one or two pages ago) I did put the address that I was advised to used this morning (they also recommended to send "to be signed for" which cost about £2.45

@Hayz0711 l just rang up for my coc scan date 18/10 they said she’s been moved from low to medium care from September when I called up off for forms

my daughter is only 2 so can’t have mobility

they said decision was made today

I’m happy but I was hopeful for high rate. Need to decide if I have the energy for MR

@LancsMcG Thankyou yeah I usually pay for tracked and signed for costs me around £8 but that’s because they weigh it too ,, I don’t think this will be as expesnsive as it’s only a few pages

Scan date 12-11 and have just got a letter for say I’m entitled to HRC and LRM same as the last two years. So surprised with being in November! I had no texts or letters before today!

Did you ring in to get the forms? If so, when?

i am the same, i just filled out as much as i could. The only other bits i added was a letter from teacher (which was the initial report for the gp for a referral) and then her SEN support plan for school which lists their observation and actions (wobbly cushion, time out, calm corner etc).. that is literally it- i did call to add that she has been accepted for a assessment. So i really don't feel hopeful either :(

@mkaislam it was like she was being funny as if he just couldn’t be bothered to go to school! I normally would, but I just haven’t got it in me at the minute.

They definitely should have an online system. Life is stressful enough without waiting years for assessments, a 5 month DLA wait and a (mostly) 2hr wait to speak to someone who then talks to you like crap!

It’s okay! That’s what mumnet is for! I’ve just filled out the form for my daughter as well. Not much to send in for her apart from her HV report which shows the result of her ASQ3 forms and her food diary which her HV asked me to complete as evidence for a referral to a oral sensory specialist.

@mkaislam it’s awful filling out the forms. I wouldn’t worry about sending enough in. I’ve seen people awarded who haven’t sent in anything other than the forms. They must’ve explained their child’s needs well on them. Something I think I totally messed up but hey ho, it’s done now.

Hi I rang last week and was told they have sent a form for school to fill in ! School don’t seem to see the same problems ! They have been semi helpful but not massively

im so worried now that this is a no !

has anyone any experience with this ? He’s 15

My sons scan date is 01/12/2023. I sent in a lot of evidence for him. And I am yet to send in my daughter’s forms. I’m sending them off tomorrow.

@mkaislam ah I didn’t realise you had 2. They seem to be getting through them much quicker at the moment so hopefully you won’t be waiting the full 20wks.

I sent in ASD report, dyspraxia, ed psych, school (can’t remember what it’s called) plan, form from school agreeing reduced hours, consultant letters and another report after his diagnosis that detailed all of his sensory issues etc. I wrote what happens day and night, but I must’ve lost my brain filling out the form because I’ve ticked no for things that are yes. Just made a mess of it.

I think your be fine, I’ve read loads of people just sending the forms with no evidence and getting awarded.. fingers crossed for us both.. keep me updated x
