Who's "fault" is this?

Person A puts things in the sink, as dishwasher is on, at some point water from various sources (cooking, tumble drier, rinsing a bowl out) was put into sink and a mug was filled at some point with water from some source.

Person B needs to take paracetamol,and decides to drink the water in the mug in the sink.

Person B starts gagging and making wretching noises and asks Person A what is in the mug, and implies that Person A shouldn't have left that water in the mug.

Person A thinks that Person B shouldn't have drunk random sink water,and should have used the glasses and fresh tap water right next to the sink.

So, was Person A wrong to leave the random water in the mug. Or was Person B wrong to drink the random water and then blame person A for being "poisoned".

Gosh jus how lazy is person 2 that they would drink a cup of water that is sitting in the sink rather than get a glass and fill it up.

I'm not entirely sure how anyone would think person 1 is either unreasonable for leaving the cup in the sink or for the last inaction of person 2. Maybe next time person 2 will be less of a lazy bones.

Did person A walk up to a sink full of dirty dishes and drink 'found liquid' out of a mug that was in the sink?!

Unless I have misunderstood, person A is a danger to themself and others if they really think they were blameless in this situation!

at my laziest I'd have run the tap to rinse out the cup and fill with fresh water.
