Hi 👋

I was hoping for people's experiences and thoughts on Create Fertility? I am considering them, so any thoughts would be much appreciated.

I am currently going through a short protocol with high stims with another clinic, baseline scan showed 4-5 follicles (expected that as I have low AMH/low ovarian reserves) but through my high stimulation am only showing 1, maybe 2 follicles that are growing...so cycle is not looking great.

So I wanted to consider Create as they specialise in natural modified and mild IVF, but would appreciate feedback on people's experiences as we would be going in multi cycle with my low AFC/AMH and want to ensure it's the best decision in those circumstances.

@HopefulElle @Jx3303 what are the timelines like during the natural modified cycles? I've only done short protocol on high stims which started on day 2 for 9 days

@Sunflower360 I should remember exactly given it was only last month, but I don’t - I started Gonal on somewhere CD7-9 and then egg collection was definitely CD15. So very short overall x

@HopefulElle yeah I think that maybe happened to me on an NHS cycle, they did no blood monitoring.

Yes I am doing 3 cycle package with the wilmslow Manchester clinic. I've done 2 of the 3 collections and a transfer (which we did as we had a good number of embryos and travel far for treatment - we have to be away from home in an airbnb during) we got 3 embryos first cycle (one high quality day3 frozen and 2 day 5s), 2 day5 embryos second cycle and we transferred one which sadly ended in a MMC we found out in May. I am getting a scan locally in my august cycle and the consultant has advised we do PGT-A testing after our cycle 3.

@Sunflower360 I have done short protocol for both cycles - started stimming on day 2 and I had egg collection on Day 12 both times I am sure. Xx

Thanks @Bubblemachine0 that is really helpful. I was on 450 IU stims for this cycle, and I am trying to get my head round how the lower stims would get follicle response if that makes sense? I definitely prefer the idea, but it does appear some Dr's tend to shut down mild or natural modified IVF as not being good, but feedback that I have heard and research, would suggest otherwise for those with low ovarian reserves

I am also on very low AMH 0.139 :( and is on my 1st try of IVF with Lister Fertility. I was given Menopur 450iu dose and has been taking them for 6 days now. Latest scan the nurse says that I am not responding still on the medication, uterus lining is still thin. I only have 4 folllicles. I am waiting for a call from the Doctor on what will happen next if they will continue my menopur for 3 more days or cancelling the cycle completely 😭

Sorry to hear that @ka3rine Have Lister advised what your options are?

I just had a call with the doctor, she wanted me to continue with menopur up to Friday next week. She said its only been a week on medication. i have 4 follicles and 1 tiny one but its small around 10mm, she wanted them to grow at least up to 16mm.

Hi All! I’ve joined Mumsnet to join this chat (I’ve been lurking during my ivf journey but felt like I never wanted to sign up as not being a mum is a sore point…but I wanted to join this chat). I’ve just signed up to 3 cycle natural modified ICSI with create St Paul’s (though reading this I need to check I was charged right!) and hoping to start in a few weeks. My background is I was with CRGH. I had my first ICSI a year ago when I had AMH 13. That was a normal stims cycle. My AFC was always 11. Got 3 eggs but 1 was damaged and 1 immature. The last fertilised to a grade B blast but fresh transfer failed. Then turned out my AMH had dropped to 4. For my next round I had max dose stims and it was abandoned due to no response. The next round I had mild stims and got 3 eggs, all fertilised but only 1 grade BC blast - frozen transfer failed. So here I am with Create. I’m hoping as I did badly with max stims, better with standard and best with low that natural
modified will be the winning formula as it’s last chance saloon for us cost wise. I’m 38 for reference and my AFC is 3-6. Been trying for 3 years and been on the ivf rollercoaster for 2. I saw someone mention Create don’t seem to know what to do about male factor. Interesting as we have that and I sent all the semen analysis to create and our consultant is discussing it with another doctor! But maybe because the various results have varied but every go before we’ve had 100% fertilised so hopefully will be ok! We’ve been given a 10% chance which is terrifying! I hope you’re all well and I’ve got my fingers crossed for all of you. It’s great to have some people in a similar position so do reach out if you fancy a chat or a vent!

Hi @Doxy21 👋 Hope you're journey with Create has been good so far. Were there any issues from your OH's perspective or was the sample looking ok? My OH's sample was good, but on the day of our first NHS cycle, we had a failed and a abnormal fertilisation of the 2 mature eggs collected, so ICSI has been advised next time round.

When do you start natural modified? I'm hoping to start when my AF comes end of this month. Did you have any other investigations done with Create? Are you taking any supplements etc?

Also @Doxy21 my AMH is now also 4.3 pmol, was 6 late last year so it has dropped a fair bit.

Hiya. So when he first had his sperm tested he was slightly low morphology but fine motility (or the other way round) and on his next scan it was the other way round. But he took generic mens conception tablets in between and his sample was good enough to try iui that time (though iui didn’t work). Since then the analysis has been from his ICSI samples and all seem ok so I guess the conception tablets work 🤷🏽‍♀️ but we have only done ICSI, not conventional ivf. I haven’t had any tests with create yet as we’ve had them all before with our last clinic. I’ve got a few next week - vit d and some thyroid ones (as I have thyroid issues). I’m hoping to start with my next cycle end of this month. Been accepted by duologi and I’m paying the down payment next week, then my nurse appointment is the week after with meds delivery. So hopefully starting week of 21st as my cycle should start that week. Are you with St Paul’s? We’ve literally been with Create since Monday. This time last week I booked a last minute scan and consultation as it was £150 instead of £450 and I was interested in the scan as it seems all singing, all dancing. Then I had an appointment with my old clinic and they suggested 2 x mild collections which cost way more than Create and didn’t include a transfer. So we went with Create as better value and we needed a chance after 2 failed rounds at the last clinic. We might be on a similar timeline? I’ve started taking vit d, folic acid, omega 3 and coq 10 again today after a break of 2 months. I was on 3 x DHEA tablets a day for my last round (though didn’t have tests for levels) and I have some left and took 1 today but maybe I shouldn’t have as from reading here Create don’t recommend? But no one asked me what I’m taking supplement wise.

@Doxy21 we may be on similar timelines, although you'll likely be a week or so ahead of me! I'm on those supplements too, not DHEA though. It may be worth you letting them know you're taking them as I think it is a hormone rather than a supplement

I only took one today so I just won’t take more. Did you choose to have your appointment in 3 weeks? Ours seems to be going ahead very quickly but probably because I said I want to start this cycle

@Doxy21 we were advised to have thyroid, prolactin, AMH and karyotype testing before recommencing IVF just to rule issues out and to decide whether to go for Mild or Natural modified (Dr advised Natural modifed based on my AMH). Just waiting on karyotype, but should be back soon to start the cycle when my AF arrives

Ah - I see. Sorry, I had to google what that test was. In my head all the tests are blood tests that they review quickly but that looks like a more involved and really useful test!

@Doxy21 I think it was advised as we've been TTC for over a year and because I'm 37. More to rule out things at this stage or to consider PGTA if something was to come up on it. I do find it interesting how at even Create clinics the advice is slightly different as I notice some people with lower AMH are still advised mild IVF over natural modified, suppose it depends on the circumstances and the Dr. I am fine with natural modified mainly because I only got 2 eggs anyway with high stims, so makes sense

Yeah. I was borderline as they saw 6 follicles but because I’ve already tried mild they suggested natural modified. But I can change to mild for some of the rounds if we decide to.

@Sunflower360 yeah I think (and hope) that they look at everyone individually, and they will be able to change your medication during a cycle if need be, or on the next cycle if they think something else is worth a try. I feel like they really want the best, it's heartbreaking to have a cycle cancelled, but as I was told, sometimes it's just not the month. It saves you in the long run given that once you've been to collection that's classed as a cycle. Of course I hope that doesn't happen to anyone, but know they they want to get the best outcome and I feel really do look after you x

@HopefulElle @VSRCM how are you getting on? I'm still waiting on Karyotype results... @HopefulElle how long did yours take? I've still got time before AF is due, albeit it's been a month since I had the test

Hi, this cycle got cancelled 😞 went for baseline scan/bloods and they said my estrogen was too low, so we should give my ovaries more time to recover and skip this one. I was told this can happen and it might not be 3 consecutive cycles & I’m glad not to waste a “go” on a bad cycle obviously, but I really wanted to get on with it! Ah well. Fingers crossed for next month.
I had a call with my doctor exactly 4 weeks after my karyotype test, the results weren’t back then and she said they should be and that she’d chase up. I got them a few days later. Give them a chase if you haven’t already, but don’t stress x

@HopefulElle sorry to hear that. Good that they are closely monitoring and as you say that cycle has been saved for a better month. Plus you have an embryo in freezer which is great too.
