For me it would probably be:

Not reading the OP properly and going off on a tangent
Pulling a thread totally OT in order to have a two way jokey conversation with someone else that goes on for pages and pages
Looking up someone's posting history and saying 'well on 5th April 2006 you said this and now you're saying that.

I do half of the above I'm sure

I get a little bit annoyed about a few of the things above occasionally - but the thing that really really gets me is text speak - there is NO need. If you can type young you can type you or your - u or ur is Just Not ON!

I have been here for bloody years and I never know what the clues are. No idea what the naice ham thing is about and they always mention that.

Some threads do make me cry though.
They're always the ones when someone has had to have their dog/cat put down or have died.
I'm far to emotional about pets though, I avoid any sad stories about them in the media.
(Cue someone saying it was just a dog, there are children dying all over the world)
I avoid these too BTW.

Posters who write loads (usually) without paragraphs, stops to thank us for reading this far and then carries on for ages.

People who type bored.com (or similar)
