For me it would probably be:

Not reading the OP properly and going off on a tangent
Pulling a thread totally OT in order to have a two way jokey conversation with someone else that goes on for pages and pages
Looking up someone's posting history and saying 'well on 5th April 2006 you said this and now you're saying that.

I can help Amber out here as I have seen non trolling derailment happen.

Self policing, when the op is trolling / goading - this chatty wavy stuff happens organically from people who don't know each other, but do know the op is as likely as a feral snail.
The sneery use these as an example of awful regulars and are wrong.

However

I have seen on one occassion a define spiteful deviation on an ops thread. None of the derailers were regs, or if they were they were under a nc. However I did and AS and discovered they seemed to have beef with the actual op and had appeared on other threads ... So I reported the Bally lot of them so I did

So if that's what Amber's referring too. Yes it exists, but I suspect it'd often people who know each other in RL being antagonising shitebags.
