For me it would probably be:

Not reading the OP properly and going off on a tangent
Pulling a thread totally OT in order to have a two way jokey conversation with someone else that goes on for pages and pages
Looking up someone's posting history and saying 'well on 5th April 2006 you said this and now you're saying that.

I haven't read the whole thread (sorry, I know that's probably top of the list) but my two major bugbears are: people wilfully misunderstanding or taking offence at an OP which clearly isn't malicious. And (usually on stepparent threads which have turned nasty) people inserting 'OP are you the OW?'. Usually accompanied by an Hmm. These things are just so unnecessary and often turn the tide of a thread which might previously have been fairly civilised. Grrr.

Definitely agree with louschmoo about people wilfully misunderstanding or taking offence at an OP which clearly isn't malicious. Couldn't believe my eyes at that thread about the heavily pregnant woman who jokingly tossed a nappy at her other half, aimed badly and accidently hit his glasses off so he shouted etc. People screaming at her that she had abused him by throwing human waste "deliberately" in his face and he should leave her immediately, at 37 weeks pregnant or whatever she was. Seriously twisting things to justify their out of proportion sense of moral outrage. Hmm
