Feel sick after first Mounjaro injection

I posted last week about starting Mounjaro and I had my first dose last night before bed. Thank you for all the advice on my previous post. I've woken up feeling really nauseous. Is this normal for the first day and will it get better? The idea of feeling like this for the next 18 months is a bit off-putting. Does it just work by making you feel permanently sick?

No, it won't be permanent. Side effects are usually associated with starting treatment and going up a dose, and lessen over time in general. It will likely last a day or two and then gradually wear off. You may have the same thing after the next injection though.

Did you properly research before you started? It can make you feel sick yes. It usually wears off for most people. No one can give you a definitive answer unfortunately as it effects everyone so differently

I have been at target for nearly two years now (went from 15 stone to 9 and a half) and I take a maintenance dose. Most of the time I don’t notice a thing but every so often I suddenly have a really upset stomach and I know it’s the MJ

just go with it and you’ll have your own experiences over the coming weeks

I’ve only felt sick once and it was after a takeaway. I’d try eating quite plainly today and see if that helps. But no, that’s not how it,works. For me, it just stopped me thinking about food and therefore wanting it. In fact, the aforementioned takeaway was ordered because we needed the containers because it’s so long since we had had a takeaway!

It affects your digestion so nausea isn’t uncommon but most people find any symptoms reduce over time and as you get used to eating to reduce them.

Dont fall into to the trap of not eating because of it, that will only make you feel more nauseous and weak.

Eat small regular meals, go high on protein and fibre, low on carbs and incorporate good healthy fats. Lots and lots of water/clear fluids. You want your digestive system to work, fibre and water will help that as well as keeping you feeling full and giving your body the hydration it needs to break down fat.

It’s not the same for everyone but no, it’s not likely to be the same every day. Some nausea or other gastric symptoms are likely at the start and when increasing dose but you’ll probably find it settles down by day 3 and if you do get more nausea next week , day 1-2 will be worst.

I started in October last year and had mild nausea occasionally, sometimes a loose bowel movement, and felt very cold through Winter. When I went up to 7.5 I found exhaustion would hit on day 2-3 and needed an afternoon nap. That improved when I started to split dose, taking 7.5 mg but in a reduced dose every 4 days.

I’m now maintaining on the same regime (not advising this but works for me) and have no side effects whatsoever.

Yes, very common.

For some people it's short term and they carry on just fine, for others it's constant. Most people are somewhere in between.

I started Mounjaro last month and felt really nauseous and weak the morning after my first injection. It subsided as the day went on, and the next morning I had breakfast before my shower (instead of after), which definitely helped.

Other things that helped: taking a peppermint oil capsule before each meal; eating small snacks (e.g. dry crackers) between meals (although I need to do this much less now); drinking plenty of water; avoiding hot drinks.

I’m three weeks in now and as others have mentioned, the nausea only happens now if I eat too much or eat something really rich.

Good luck - I hope you feel better soon.

Hi @LabOwner95
I am a couple of months in and still have half a day of nausea a week. I take cyclizine, prescribed by Voy, which does help. This is my second time trying jabs (gave up due to nausea and diarrhoea) so I chose a pharmacy who provide cyclizine. Your GP might also prescribe it.

Yodeldodeldo · 02/09/2026 09:14

Yes its normal, you'll probably adjust. I found the occasional mint helped.

Katalica · 02/09/2026 09:33

Every body reacts differently so it’s important to track what foods you’re eating just before, during and after jab day and then you’ll know if certain foods are causing you issues, rather than it just being the jabs.

Assuming you’re trying to improve your overall diet, focus on meals with high protein and lots of fibre and try and avoid fatty and rich carb heavy meals as they’re known to cause issues for many people. Also, it’s very important to drink lots of fluids.

I think some nausea after the first dose can happen, especially while your body is getting used to it, but it shouldn’t mean you’re expected to feel permanently sick. I’d keep meals fairly simple for now, drink plenty and see how you feel over the next few days, but speak to your prescriber if it’s severe or doesn’t settle
