Mounjaro + mirtzapine / amytriptaline

I saw a thread on this a few months ago but thought I’d gather more first hand experiences! Has anyone who takes both a jab and the anti-depressant (either mirtzapine or amytriptaline) notice the weight gain or loss it has had on them?

Been on Mounjaro for 9 months and after a massive weight loss in relatively short time when I started (I paid for it) I’ve been steady slowly losing for the last 5ish months and lost about 4+ stones and was nearly at the 30kg loss mark. Then started retaking first amytriptaline slightly more than a month ago and then switched over to mirtzapine 2ish weeks ago and I haven’t stopped eating. Cravings have been full force (eating things I’ve not touched or even been inclined to touch for 9 months) and I’ve regained 3-4kg in the last 2-3 weeks alone! I take the meds for anxiety / insomnia / OCD but feel it isn’t worth the sacrifice anymore if it’s causing / cancelling out the effects of MJ. I’m on 12.5mg of MJ atm (have been for 6ish weeks) so my appetite should be well suppressed! I’m moving up a dose but curious to hear what other people’s experiences have been as I would’ve ideally like sitting on the 12.5mg for a bit longer.

I will note that the effects of MJ does tend to reduce after being at the same dose for 2-3 months (I think my body habituates to the meds slowly in that time and overrides it eventually) but this is way quicker then I expected to feel cravings more obvious. A bit disheartening so any insight/advice or shared experiences would be appreciated!

I'm on mirtzapine but had to quarter my dose after being fine on it for over a year. I think I'm 7.5mg of Mitz now because it's suddenly started wiping me out. I still have low desire to eat. And awake! What dose are you on? I've been told its a sedative. Amatriptalune I have only ever taken for migraines. I "think" they qork via different methods.

Mirtazapine is an AD given to people with eating disorders - but I think the low dose makes you hungry and the higher dose not so much and low dose makes you sleepy -I had it for insomnia

I'm on amitriptyline and haven't noticed any interaction between that and MJ.

interesting maybe it’s cause I thought maybe it was because I was on a higher dose but the other comment suggest it might not be so I’m a loss now :/ I’m taking 30mg of Mirtazapine at night, and with the amytriptaline it was 2 tablets so I think 20mg.

I read this online as well which is why I thought I’d see if perhaps others had similar experiences with this possible nulling the effect of MJ 😩 Now I don’t know anymore what to think, just that the 30mg works in tapering off nerves and sending me off for at least a few hours of sleep at night but not nearly enough to keep me asleep whole night. But don’t want to undo my hard work of losing weight if it is messing with the effects of MJ 😭

The MJ might be affecting the absorption of Mirtazapine

Buffyj · 21/08/2025 09:54

Mirtazapine is an appetite simulant. One of the biggest side effects is weight gain. It can lessen/prevent the effects of mounjaro. Lots of pharmacies won't presribe Mounjaro if you are already on Mirtazapine as it is a waste of money.

Time4changeagain · 21/08/2025 13:18

I was on 30mg of mirtazapine a night for years for the same reasons as you. It helped me fall asleep but not stay asleep, I was also so groggy during the day. I would wake up in the early hours and need to eat. I put on a massive amount of weight too.

This is my own experience & I’m not advising anyone to do the same

I had been on MJ a couple of months and read somewhere on here about what it was used for and decided myself to stop it. I started to sleep a bit better & I didn’t feel groggy during the day. My sleep is still crap but the hunger has gone.

I'm on mirtazapine 15mg and have gained a lot of weight while taking it. I was concerned before starting mounjaro that it wouldn't work, that the appetite stimulant effects of mirtazapine would cancel out the effects of the mounjaro. But it's been fine and is working well, I had reduced food noise from day 1, the crazy mirtazapine food cravings have completely stopped. I've lost 37lbs so far, I'm on week 16.

Thanks everyone for your insights! Very insteresting to get different perspectives of how everyone may have responded to the combination of these two medications. I really cannot attribute my current weight gain and influx of hunger to anything except this change in medication :/ it’s just to coincidental and have not change anything else habit wise in my routine, so it may just be I’m one of the unlucky few who may be adversely affected by this combination but time will tell if this is the case as I’m going to come off it, I hope my sleep improves on its own. I don’t want to jealordise my progress, so I hope I can see if this brings back any of my suppression as I let the AD medication clear out of my system 🥲

again much appreciated everyone!
