I have some health issues so my doctor wants me to start in a half dose, which I understand and am on board with. Just to see how I tolerate it.

However this will mean that a pen will last for 2 months. Is that ok? I can’t justify throwing it away after 30 days half used

So long as you store it in the fridge there is no reason that it won't be fine. Lots of people micro dose.

I am not medically trained to give advice, but I am sharing my experience.

I keep pen in fridge at all times.

To start of with…
my first 2.5mg pen, I stuck by the rules and had 4 doses.
there after, I took the golden dose so minimum 5 doses apart from pen 3, where I was taking 3 mg from a 2.5mg pen so I only got 4 doses.
Pen 4 & 5 is from a 5mg dose pen but as i was taking less clicks, I didn’t want to risk going over the 5 weeks so 2 doses were wasted from each pen.
Pen 6 is from a 5 mg pen but I decided not to waste any doses and used pen 6 for 7 weeks with no ill effect but I only lost on average 0.5lb per week .
Pen 7, I have upped the dosage to 4 mg from a 5mg pen. My loss so far with this pen is 4lb in 3 weeks. There is enough doses for this pen to last 7 weeks which I plan to do.

Pen 1, 2.5mg over 4 weeks - loss 14lb
Pen 2, 2.5mg over 5 weeks - loss 8.3lb
Pen 3, 3mg over 4 weeks - loss 5 lb
Pen 4, 3.25mg over 5 weeks - loss 6.5lb
Pen 5, 3.6mg over 5 weeks - loss 6.5 lb
Pen 6, 3.75mg over 7 weeks - lost 4 lb.
Currently on week 3 of Pen 7, plan to take 4mg over 7 weeks, currently lost 4 lb

Total loss over 33 weeks is 47.5lbs or 21.5kg

To work out clicks…
Dose required, divided by pen dose x 60 = clicks
so currently taking 4 mg from 5mg pen =
4 divided by 5 x 60 = 48 clicks.

If you wanted 1mg from a 2.5mg pen it would be
1 divided by 2.5 x 60 = 24 clicks

Same theory regardless of dose required or pen dosage.

You might only take 6 doses if you move up to the regular dose after 4 weeks on half. 6 doses is within 35 days, so it might not be in use for 2 months. (I'm perfectly happy to push it to 35 days when kept in fridge with good hygiene practices, etc.)

Are you ordering privately? If so, be aware that they may be reluctant to prescribe the next dose pen if you have a larger than 4-6 week gap between orders.

In that case, you should follow their directions. They may advise you to use a pen for longer, off-licence.

Even if you have to throw some medication away, it isn't really a loss as you will still have been medicated for the full 30 days on the cheapest available pen.

I would be wary of going over the 30 days and extremely concerned by any professional advising you to do so.

It’s not just about temperature control. Every time you add a needle or withdraw a dose you pierce the bung on the vial and risk introducing bacteria. Using a clean technique, wiping the end with an alcohol swab (and allowing it to dry for 30 seconds) will reduce the risk of introducing infection - but it remains a risk and is the main reason a vial should not be used beyond 30 days.

Making a pragmatic decision as a user to go a day or two beyond 30 days is probably OK, but no professional should be telling you to keep a pen beyond the safe limits.

No-one can credibly tell you if it is "ok".

The manufacturers of the medication will only say they have tested the in-use period is 30 days and the pen must be discarded after.

Everything else is people taking risks that they feel comfortable with. It is unknown if it is just the effectively that reduces, or if/when harmful bacteria would be present or if the compounds in the medication degrade after the seal is broken and exposed to air and become harmful.

No-one can tell you if your pen, its storage and in-use conditions in a domestic environment and in your body is safe beyond 30 days so don't listen to anyone who says it is safe as they don't actually know that (unless they work for Eli Lilly!)

You need to decide for yourself what level of risk you are willing to take, as only you will face any consequences.

With 5 doses possible in the 30 day in-use period you need to reframe seeing throwing away medication as a loss. You have already got a week more treatment than you paid for.

Ask yourself if you would inject your child with medication significantly outside of its in-use period? Most wouldn't take that risk. Your own health is just as important.

Remember also if you are messing about with doses, each dose builds in concentration in your body until it levels out after 4 weeks, so changing doses more frequently than every 4 weeks could leave you not knowing what dose level actually caused side effects as it hasn't been approached following a dosing schedule.

If your Dr is telling you to take, off license, a half dose they should be prescribing with a dosing schedule and clear instructions how to do it. If they haven't I would question their professionalism and knowledge of the medication.

If your doctor is prescribing MJ for you at a certain dose it’s up to them to ensure your prescription matches. They absolutely cannot just tell you to take less for longer.
