How to use the "bonus" dose

I'm on my 4th week of MJ and it's not really working now.

I've just injected for my last week before going to 5, and wanted to use the "bonus" dose.

But I can't get it to "click" or turn round to be able to administer it.

I think there's still a full dose in there. Any ideas how I can turn the "dose knob" to get it to work.

And the graphs are lovely, although didn't heed one part of your advice. ;)

I think we all know we should ask our prescribers but some of us (me) just do our own thing!

So I've had no bad reactions from taking 5ml and can still eat heartily.

I've had 3 scrambled eggs and 2 slices of sourdough for brunch and a large baked potato with cheese and salad for dinner.

So not made a dramatic difference to appetite suppression, although craving for chocolate seems to have subsided. Which was/is my main problem.

I think, if you are going to use the fifth dose, a syringe is a safer option because, once you have "broken" the syringe, you cannot guarantee it is going to continue to measure correctly.
Obviously neither is recommended by the manufacturer

Yes definitely. Too much room for things to go wrong when breaking the pen. It’s not worth it.

Haha that was me.
I think I ended up having closer to 7.5 as I had more left in the pen than normal for the 5th dose as for the bonus dose on 2.5 I wanted to be 5mg so I took 2.5mg from the 5mg pen.

So the 1st dose on 5mg there was only 0.3ml used.
Which meant the bonus dose on the 5mg pen had what ever would normally be left over in the pen on a bonus dose plus half the dose that was left over from the 1st dose if that makes sense.
I have probably caused confusion but safe to say I won’t be breaking the pens again!

Tbf an extra 5mg is another week for me, I've stayed on 5mg for a few months and it's useful to get 5 weeks rather than 4 out of a pen.

Not sure if anyone has tried it, but I've read in a few places now that you can get the 5th dose out without forcing it by stopping your 4th dose just shy of the 1 on the pen.

i.e. when you take the 4th dose, you only click round 59 clicks so that you are 1 click away from the 1. Take that as normal. Then, when you come to do the 5th dose, you can click round to the 1 as normal without the need for force.

Has anyone tested this - or wants to? Grin

I have just done my 4th injection of 2.5mg and have reordered more but at 5mg. I want to use the extra dose as there looks like there is loads in the pen !!

I am taking 3.75mg from a 5mg pen (45 clicks).

My 5th dose clicks up to around 23 clicks on my fifth dose then stops and I need to break/force it the rest of the way up to 45 (I mark it with a ballpoint pen on my first dose).

I think the pen is setup to allow a total number of clicks (around 250ish) rather than a number of doses.

Some pens are easier to force than others.

The first one I tried was so easy I was wondering what everyone was on about that it was difficult. I have no upper body strength so was amazed.

The second one however was much more difficult. I didn’t think I was going to manage it when it went. I just can’t bring myself to use an insulin needle so it’s the only way for me

I have huge block with the insulin syringes too - I bought some and opened one for a look and can't get my head around 8mm instead of 6mm (it looks A LOT longer on a syringe!) and then seeing the zoomed in pictures of a blunted needle after one use (going through the red seal) finished me off!

I know those who have done it say they don't feel it, I know I don't need to insert the 8mm the full way, but that would mean LOOKING at the needle going in when I am injecting and there is no way that will happen...

I've needed to use pliers on my pens, but they do move and I haven't had any problems pushing the dose knob down to administer so will stick to that and will probably have to bin any that don't work.

Your thinking is exactly the same as mine! It looks like a needle and it’s just a step too far for me. I can inject the pens so problem at all but nope I just can’t get my head around insulin needles.

The hard twist might be easier than you think. I’ve managed it and I can’t open jars 😂

Don't know but it sounds far too complicated to me ;)

I genuinely didn't think I'd be able to twist it off, took me about half an hour but I got there.

As a PP said, it feels like you're going to break it. And it doesn't click it just twirls round.

@Davros on the Continuing Mounjaro threads in the first post there are links to information about all of this.

My understanding, having never done it, is that you use the insulin needle to pierce the red seal at the end of the Kwikpen and extract the remaining liquid that way. You can then see how much in ml you have in the needle and can calculate what dose that is from the ml measurement.

I did hard twist previous month and using syringes much easier plus you know how much. I got .7 out so a whole dose plus tiny bit

Beware of purchasing syringes from Amazon. You'll notice they're called a measuring tool instead which means they may not be sterile or fit for human use.

You're better off buying from a pharmacy.

Just a quick update after using bonus dose. So didn't have any side effects at all, but only lost 1lb that week.

So total in a month is 10lbs, which I'm happy with.

Now onto the official 5ml dose so hopefully will start feeling suppression now.

So I read somewhere that if you give an extra click on your first dose you can get use of the bonus dose.

Anyway that totally buggered up my syringe thing and now it won't plunge.

So have ordered some insulin needles. How do I draw it from the syringe thing?

For anyone interred, I'm at 15lbs down now in 6 weeks. This is my first week of 5ml (prob bit more as got wee bit of bonus dose out) before I broke it) and at last have some appetite suppression and don't have to take it every 5 days!
