Google, were you hay with your cbt therapist? Have you asked on mn how to cope with panic attacks? You do have my deepest sympathy - I know what they are like. I used to run out of shops halfway through a transaction (this was some time ago) sweating and heart beating.
Last time I had one, and it was the only one in years, was about 18 months ago at the dentist before some fairly major work. I started hyperventilating, and he told me that unless I stopped he wouldn't treat me. Well, that was urgent enough for me to force myself to do the textbook breathing through the nose, etc.
I also did a year long counselling and counselling skills course, mainly motivated by wanting to know what a therapist would do if I paid them. That was very very useful, but a bit drastic, perhaps. And getting older helps too, I think. You can learn to deal with them in some ways.
I am very cynical about some schools of therapy, but cbt and 'short, solution-focussed therapy do seem to work for relatively well documented and manageable problems.
I hope that doesn't sound harsh. I do think that a lot of your suffering could be managed, and I am not sure that your cbt therapy has been effective.
