Utterly clueless about DIY - help

I bought my flat last year and live on my own with my 3 yo DS. I have got no idea about any DIY whatsoever, can barely change a bloody lightbulb.

Now I would like to spruce up my (tiny) kitchen. Namely I want the worktops including the sink replaced. Possibly also a new floor, and the whole thing painted. The painting and the flooring is easily sorted, but how the F do I go about replacing the worktops? Will kitchen places such as Wren etc do a worktop-and-sink-only fitting? I don't need any new appliances.

Budget is not hugely important to me, I'd rather someone do the lot for me rather than save a bit of ££ by faffing about measuring and doing everything myself...

Where do I start?
