Done, but the formatting was odd on an iPod so the multi-choice boxes wouldn't tick.
Not a problem as they were mostly "not applicable", but thought you'd like to know.

Shame there were no options to tick between 'being on a diet' and 'not being on a diet' as I'd say a lot of people just aim to eat healthily!
