ERA testing after multiple implantation failures. Anyone had it done?

I've had four transfers so far (first 3 from 1 batch, fourth from a second egg collection) and 4 BFNs. I have a naturally conceived 4 1/2 year old and had a natural pregnancy with early miscarriage a bit over 3 years ago. My lining struggles to get to thickness, I have polycystic ovaries (on sukkarto), a fibroid, slightly high thyroid thingy (on levothyroxine), high bp (on meds) and a midline septum (ridge/divide in the uterus). I had surgery to unblock a tube 2 years ago. I'm in the healthy weight range.

My clinic have suggested PGS testing (we have 5 embryos in the freezer) but I'm really not keen on it as I've read about actually fine embryos being shown as poor and therefore being disposed of.

They also suggested ERA (endometrial receptor assessment). Has anyone had that done and found it useful? I just hate anything that delays the next FET but I think maybe it will be worth it?

Any other suggestions? Last FET I was on lots of estrogen, extra progesterone injections, clexane, neupogen infusion, aspirin and probably a whole load of other stuff I've forgotten.

I am so fed up with it all!

@Cream123 oh no, I've heard anecdotally that FRERs can be a bit inaccurate once you've reached the point of testing positive on other tests and that blue like tests are better. Can you try one of them?

bluepixie · 29/05/2021 08:19

I’m gonna try get a private hcg and then another in 48 hours (though it’s bh)

Not sure what to think as I’ve never bothered testing twice before on a frer don’t know why I did ! I’m usually having hcg monitoring or end up having a late miscarriage (I’ve had 1 chemical before but not with a pgs embryo)
My Head is spinning :(

The line is very obvious. It’s just not darker

Gardenlady543 · 29/05/2021 08:51

@Cream123 that's a good idea, will your clinic do it for you? Mine is open on bank holiday Monday with a skeletal staff.

bluepixie · 29/05/2021 14:59

Don’t really want to hijack this thread with my worries! Here to cheer u both on!!! So far the lack of line progression is not good.
Got a hcg but my next one isn’t for a while due to Bh.

Gardenlady543 · 30/05/2021 09:46

@Cream123 I hope you can get some clarity soon.

I'm hoping my scan goes ok tomorrow, I'm feeling so worried, I hope it helps. It's feels like I'm starting from the beginning right now after 18 months of stress.

bluepixie · 30/05/2021 12:49

I’ve just started bleeding :(
My hcg was 88 which was pretty good but this is too much for an implantation bleed
Sigh.

Good luck tmrw @Gardenlady543

Gardenlady543 · 30/05/2021 13:04

I'm really sorry @Cream123 that's awful Sad is this what has happened in the past? you mentioned you've been pregnant quite a few times since your first?

Can you book in with Mr Lower soon so he can advise?

bluepixie · 30/05/2021 13:21

No all my mc were before him. I had 6 (4 after heartbeats and 2 chemicals). Then I had my son. He was my first pgs transfer with immunes so through we cracked it.
Then after him I had 2 negative ivf cycles which was weird given how easily I got preg before - diagnosed with ashermans. Had that fixed as best as can though things are not great inside. And now this chemical.

All my other mc were later on and so were my 2 chemicals I never actually bled with my chemicals my hcg just started slowing down and then stopped so I stopped my progesterone and it all ended.

Gardenlady543 · 30/05/2021 13:54

Oh that's interesting @Cream123 it sounds like it's worth carrying on the medications and getting the repeat hcg. I know my clinic tell me that I should carry on at least until the OTD even if I bleed. At the very least it sounds like a positive that you've implanted if that hasn't happened since your first child. Obviously even better if you get a hcg rise.

bluepixie · 30/05/2021 14:00

Yes def second best case. I just hope it ends sooner rather than later as I don’t think my uterus will manage any op for a mc I will certainly get ashermans again

I’ll keep going and ring care tmrw and plead for a hcg with them.

But for me any bleeding in pregnancy is a bad thing.

Gardenlady543 · 30/05/2021 14:19

It all sounds really stressful @Cream123 I have my fingers crossed for you. And I hope they can get you in for a blood test tomorrow so you'll know more.

bluepixie · 30/05/2021 16:28

I’m having one on Tuesday with care.
Good luck with ur scan @Gardenlady543

seven201 · 30/05/2021 22:20

Oh I'm sorry @Cream123 . You must feel so stressed out and bereft. I'm glad you at least have the hcg booked in. It's the not knowing which way it will go that makes it even harder. I don't think you should factor in the line not getting much darker if it was already a prominent one. You're not out yet, although I do appreciate it's not looking hopeful, you just can't know yet. Big hugs to you Thanks

@Gardenlady543 good luck for tomorrow.

Oh I had already mentioned the bleed. Brain all over the place. Anyways thanks for ur support
Filled with dread doing this alll again! One more blast left

Gardenlady543 · 31/05/2021 09:20

@Cream123 I hope the blood test shows everything is ok, not long to go.

I had my scan today (day 11) I'm at 8.2mm, I'm usually at this point by day 13 and she said the lining is trilaminar and looks good. She didn't realise that my specialist wants me to get to 9mm and my specialist seems to have missed this off my paperwork.

I have had a fluid collection around my left ovary since after my fresh cycle. The nurse wants to rule out it being a follicle. So they did a blood test and they'll call me this afternoon. She said they may bring me back in for another scan, which should be happening anyway since I'm not yet at 9mm. I'm sure I'll have to fight just to get them to set the criteria at 9mm because the paperwork doesn't say it.

When I told the person taking my bloods how stressed I am and how I don't know what I'll do if it doesn't work since I've had so many tests already. She said, you realise that it sometimes doesn't work even with everything right. My specialist has always tried to get me to think positive, I didn't realise need to be told to expect to be disappointed when I've never had a BFP ever.

bluepixie · 31/05/2021 13:22

8.2 is amazing ! Not sure why u have to get to 9? Fingers crossed it will get there anyways...
What a stupid thing to say! U really you don’t need someone telling u this!

Just stay strong and hopeful. U have lined up so much more this time with ur era and micorbiime and ur lining sounds fab.

When do u think transfer will be ?

bluepixie · 31/05/2021 13:27

Oh U want to get to 9? I think 8.2 trilammainr is great if u don’t make it but fingers crossed U will :)

Gardenlady543 · 31/05/2021 14:16

@Cream123 in my first FET at the time of the transfer the lining was noted to be very thin, I'd got up to 8mm before starting the progesterone so it seemed to have compacted a lot, my specialist put the 9mm minimum down to try and compensate for this when we did the mock cycle. And I mean given the lining issues and I know I can get to that thickness I guess I'm happy with the thicker the better, I expect it will be at 9mm by Thursday.

Just had a call from the clinic my oestrogen blood test has come back at 8000 😲 it was 600 in my first FET. I am on a higher dose of estradiol but they're worried that the fluid around the ovary is a developing follicle, so I'm back in for a scan tomorrow. The nurse confirmed that if it is a follicle they'll convert to a natural cycle. I have no idea what that might involve with me already being on meds.

How are you feeling? Have you had any more bleeding? Have you done anymore tests at home?

bluepixie · 31/05/2021 15:49

Oh god 8000!!!! Glad they picked it up...I’ve no idea what happens when u convert to a natural either! I hope ur ok with the uncertainty

No more bleeding. Just had one gush yest which was red. Then rest
Of night was brown on wiping and brown when I got up this am . After this am not even got brown. I don’t have cramps. But I lost my funny taste in mouth the day my stick didn’t go up. So I’m pretty sure that coupled with the bleed mean it’s all over. I would love to be wrong but I feel like the expert miscarrier and I know when it’s going wrong. Just hope it doesn’t drag on. I’m glad it means lower has massively improved my uterus and there’s chance to get pregnant again but obv it’s very confusing when u do PGS and immunes and an era and microbiome and it still doesn’t work....not sure wot I’ll do next. May have to add IVIG and pred like I did with my son and I was trying avoid. As well as another hysteroscopy

Gardenlady543 · 31/05/2021 17:16

I really hope you're wrong @Cream123 but if it is bad news then I hope Mr Lower can see you soon and advise. I hope you can have as relaxing evening as possible.

Do you think this super high oestrogen could be causing me to feel really anxious. I am not an anxious person, but it feels like I'm peering over the tallest building imaginable (I'm scared of heights) 24 hours a day.

I don't really understand it, usually I've started off on 2mg 3 times a day orally and then they've upped it by an extra 2mg twice a day (there was also a patch in my mock cycle, but I don't think it did much). I can't understand how my level is 10 times higher than last time when I'm pretty much on the same dose I was on after the increase in my last 2 cycles. I can see how it might be twice as high, but 10 times as high?!

bluepixie · 31/05/2021 20:53

The oeestogen levels r similar to a fresh cycle - it’s usually about 1000 per egg. So it’s utterly crazy that yours is 8000! Mine is 1300 on a patch 3 oral and 2 vaginal
Tablets...so I wonder if it’s like a function cyst I don’t get it?! They can check ur bloods on day 1/2/3 to check all Quiet IF they need to in future...(obv this will not be needed!!!)

I think the impending transfer is enough to make u feel anxious :( if I’m
Honest. But all the drugs and hormones running around def make me a bit more unstable!

Hcg tmrw at 8, then the longest day ever to wait !

Gardenlady543 · 31/05/2021 21:01

@Cream123 you know the answer to everything! Smile ok this makes absolutely no sense to me then, if there is 1 follicle then it can't be contributing that much and 6mg estradiol orally + 6mg vaginally, really isn't a huge amount to be on.

They call the fluid a paraovarian fluid collection, it's outside of my ovary and they haven't been at all concerned about it before now. This is all really strange, I hope it doesn't mess this cycle up. I'll be so upset if they terminate the cycle. Why do I need to have a new problem arise each time I think I've resolved one!

Ah bless you, it's good that the blood is being taken so early, do you know when you'll get the result back?

bluepixie · 31/05/2021 21:12

I’ve been through so many ivf cycles and hcg tornados I should make it my full time job! I’m also obsessive and research till blue in my face but only because that’s the only way I got my son. I got so much shit advice along the way and I think u have to advocate for yourself in fertility - like u pushed for ur own era Emma etc. Can ur doctor ring u tmrw and get an explanation. U don’t want it cancelled but u also don’t want to waste a blast. So just don’t be hasty and do what’s right after ur consultant advises u. Paraovarian fluid collections I’ve no idea about!!!!

But yes very confused why it’s so high?! Makes u wonder they didn’t mix it up with someone else?!

Hoping it’s back by early afternoon I’m at work but I’ll just take the result at work. Need to be 200 plus really to be a decent rise. today toddler asked me for a baby :( . Sigh.

Gardenlady543 · 31/05/2021 21:19

@Cream123 my specialist only works on Thursdays and Fridays unfortunately and it wouldn't surprise me if she's stumped too. I think I'm her most confusing patient with my progesterone hypersensitivity, weird patchy lining that was a mirage and the world's worst microflora.

They have a meeting every day and whichever doctor is there will advise. The person that took my blood was very thorough about IDing me so if there is a mix up then it would have been at the lab end or when they were sorting out the afternoon patient phone calls. I guess I'll have to see what happens tomorrow.

I hope it comes back quickly and that it's good news for you tomo.
