Strange smelling penis

Hi, promise I’m not a troll but this feels more health issue than sex topic.
My dh has had a problem with a strange smell around his penis area for around 4 years.
It seems to be around the head area and all I can compare it to is a fishy smell. It’s really starting to affect our sex life and his confidence in general.
So far we’ve tried...
He has always showered/bathed at least once a day, he has increased this to 2 times but made no difference
Used ph sensitive wash
Had swabs/Sti check - all clear
Gp referred him to urologist who checked with a small camera for bladder infections etc
Investigated for trimethylaminuria (searched the issue on mumsnet and that was something someone else had)
Tried thrush creams
Currently trying antibiotics as the gp is now at a loss as to what it could be
Possibly tried other stuff I’ve forgotten.

It’s bizarre as it can be there one time we check then possibly stronger 1/2 hour later then an hour after it’s gone.
The only thing we can think that possibly could be a factor is he sweats a huge amount.
I know that people may come on to suggest he’s cheating or whatever but I’m 100% that he’s not.
We are so stuck with what to do, the gp is stuck and so I’ve suggested asking on here and we are now at the point of what else can we do.

He’s had urine tested a few times. Definitely going to push for the bv tests thank you so much. Our doctors are lovely so hopefully will investigate

My exes pee used to make him smell awful and it was because he was on anabolic steroids. It was so strong.

OP does he consume a lot of protein drinks? I find they can make guys smell "off" (but not usually fishy).

Protein does make you smell but erm it’s usually arse (protein farts are a thing no one should ever have to experience)

I’m at a loss - what about trying a food diary to look for connections?

No he stopped using them a while ago, has tried cutting out eggs and things too

Does he use shower gel with exfoliating grains in it? Once my DP announced I had given him thrush. Considering I didn't have thrush he worked it out that he used the above type of shower gel and it caused irritation.

I can remember reading that an inflamed prostate can cause semen to smell fishy. Could this be a possibility?

I’m not sure if they’ve maybe checked the prostate when they did the camera.
It’s not the semen tho it’s more like his skin there

Has he tried a Change of diet?

We were convinced it was trimethylaminuria but he had a test although the go only took 1 urine sample and I’d read it should be 2 and after reducing or increasing the foods you mentioned.
Anyway the test came back negative but I did say to dh that I don’t think it was carried out correctly

If you think the trymethylaminuria test was carried out incorrectly he could just cut out all the food associated with making it worse and see what happens. So it for a good couple of months, really thoroughly and if the smell goes away then you have your answer! If it doesn't go away then that's also pretty conclusive that he doesn't have it.

If he tries not to eat the foods that are known he'll need to avoid chocolate as I think it contains lecithin as an emulsifier. Could be wrong, but it rings a bell somewhere.
