Reaching out for help or advice - adult ADHD?

Not sure how to start this so if I jump in and write a list of the problems I have day to day could anyone with any knowledge tell me if they think this sounds like ADHD please?
All are issues I've had for as long as I can remember that get noticeably worse in times of stress. I'm mid 30s.

  • persistent lateness/problems with time management
  • massive problems with both short and long term memory even significant things
  • difficulty in prioritising mundane things
  • difficulty meeting obligations (remembering to return school letters, important tasks or appointments etc)
  • never meeting full potential through failure to follow things through/procrastination/inability to organise myself
  • constant feeling of choas (inability to recognise patterns in things so for example DP would notice that me or the kids always do X after y but I wouldn't realise)
  • inability to concentrate when there's any background noise
  • often lose thread of conversation half way through or tune out when others are talking
  • interrupting people in conversations without meaning to
  • useless at household organisation
  • incredibly low self esteem resulting in bad relationships
  • inability to get organised or whatever enough to stay in touch with people even when they're really important to me like lifelong friends (who are thankfully very understanding of this and are busy enough to not take it personally)
  • inability to form proper routines which would help but never seem to stick
  • constant forgetfulness (why did I walk in this room?/where did I put xyz?)
  • either completely focussed on something to the detriment of everything else or the complete opposite

I'm sure there's more and this stuff can be normal at times but I'm always like this and I really need some help.

I've made a drs appointment (I went years ago but Dr just told me to self refer for counselling and seemed to put it down to stress) but I'm worried that they'll just think I want an excuse or a trendy diagnosis or something.

This can't be right though can it?
