Crisis team after discharge from secondary care

Does anyone know how long it is before someone from the crisis team gets in touch after being discharged from hospital? I am worried I may fall through the system and get missed when I really can't afford to.

Hi, they should contact you within 24 hours. Well that's what it's like I'm my area. It should be discussed with you at your discharge meeting.

I've always seen someone from the CT just before I've been discharged and they've orgqnised the appointment then for the next day.
