My isopropyl is 99.9%

So for every 100ml of 45% hand wash I add say 20mls of isopropyl to make it 65% alcohol?

It wouldn't be 65 as the volume would have increased to 120 so you'd have 65/120 alcohol. You need to add at least 40, giving you 40+45=85 per 140 which is just over 60

So to 100ml of hand sanitiser I add 40ml of isopropyl?

At a minimum.

Antibacterial handwash is effective for bacteria. Coronavirus is a virus not a bacteria so soap and water will do the job.

Hot water and soap with the correct hand washing for most people is good enough.If everyone else who still feel that they need hand sanitizer.You can make your own.The internet has recipes from hospital sites.There are numerous sites that sites selling hand protection gels.At first my answer may may seem harsh but I think that there are many carers ect are finding it hard to get hold of the products my brother in law has been waiting for a lung transplant not through he done in his lifestyle he only in his forties and had just has a fluke genetic disease only found out by a curious doctor and nurse at their local G.P.surgery before this he was told it was asthma.Please note that there are very vulnerable who are trying to stay well.Take care everyone and stay well
