cory - the questionnaire used was the SDQ tool, and although we can't actually read what it says on the web, I have some knowledge of it as I was indirectly involved in some research that used it. It's a pretty robust questionnaire, well-validated, and it's written to try to get over the problems you outline. The researcher who used it on the project I was connected to was v. experienced and trained specifically to use it.

This is not going to overcome all the issues of subjectivity and personal interpretation, but it goes some way to doing so.

And my oldest was only breastfed for a short time as I had to stop breastfeeding (having had a ceasearean and then having a heavy period every week on the mini pill I had to switch the the combined pill and bottle feeding) but he is very well behaved both as a baby and ever since- he has a lovely, helpful nature and rarely ever needs telling off in fact.
