Very low Papp-A and very high Bhcg result

Hi. I’m new here and frantically trying to find some hope. I’m 37 (38 on Tuesday) and expecting my second child, we went for our 12 week scan on Monday and had the screening test done. The scan seemed fine and said I’m actually 13 weeks, not 11+5 that we figured from my last period. Got a call the next day saying I’m very high risk for Down’s syndrome. Went back to the hospital and said I’m greater than 1 in 5 chance so anything up to 1 in 2. Devastated isn’t the word. My partner said he wouldn’t want us to continue with the pregnancy if it comes back a certainty. He has 4 children and doesn’t think it would be fair on our 2 year old, not to mention the baby :(

Actual results:
NT- 2mm
Bhcg- 3.68
Papp-A- 0.23

The midwife said it’s the gap between the 2 numbers that’s the biggest indicator. Anyone had such high/low blood results in my age range and it’s been ok? 1 in 2 to 1 in 5 sounds a lot higher than a lot of the threads I’ve found on here but have seen some positives for the 1 in 5, however she was very specific about mine being GREATER. I’m so lost. There was no mention of the nasal bone and sonographer said everything looks ok.
had the NIPT test done do now I’m in this agonising wait.

Thanks for reading x

The NIPT is very accurate so I’d wait on that, then if you need to go for the amnio. Remember there’s still a chance everything will be fine. So you know how long you have to wait for the results?

KellyMarie84 · 21/10/2022 11:29

Yeah, next Friday they said. I just have a feeling it’s not going to be the result I want x

I has a similar papA but lower back reading and I was 43. Scan all looked fine. Think I was given 1 in 75 risk. Thinking of you.

KellyMarie84 · 21/10/2022 12:47

@Mmr224 what was your outcome? The midwife said it was cuz of the high Bhcg Vs the low Papp-A it’s hard to feel optimistic at this point. But I guess there’s still a small chance i could be one of the lucky ones. The wait is torture but dreading the call as well x

Mmr224 · 22/10/2022 01:29

We were lucky and everything was fine. Wea actually had a higher nt measurement although still in normal range. Thinking if you as the wait is horrible.

KellyMarie84 · 22/10/2022 10:59

@Mmr224 do you remember what your numbers were? I’m in a living hell it’s awful. People with my kind of results aren’t appearing to have good outcomes. Was there any other issues with your little one following such a low Papp-a result? X

OP posts:
darklight03 · 22/10/2022 14:19

I’m in a very similar situation but higher NT. hcg 1.88, pappa 0.36 and NT 3.2 giving a 1 in 2 risk. Also second child and 35 years old. Beside myself knowing what’s going to happen, just feel that it can’t be anything other than a bad outcome

darklight03 · 22/10/2022 14:20

My scan also put me about a week ahead of what I was expecting

Passanotherjaffacake · 22/10/2022 14:27

I wasn’t told my numbers OP but my result was 1 in 42. I’m the same age as you and my HCG was high and my Papp A was low. DS is here and fine. We relied on the NIPT. The wait is terrible.

My hcg was high and Papp A were low for my first baby but the extra three years on my age were the difference between 1-1036 and 1-42.

the fact your scan looks good is great. My sonographer said scans are really good now so they can often spot problem. Not sure if she said that to reassure me but I found it helpful!

apparently that is just my profile when pregnant.

I’m sorry you are going through this, it is horrible. Xx

KellyMarie84 · 22/10/2022 21:49

@darklight03 im so sorry you’re going through this too :( I’ll be thinking of you. When are you due your results? Your NT is still in normal range though I believe and your hcg is half of what mine so I’ve got everything crossed for you. Such a worrying time though x

OP posts:
KellyMarie84 · 22/10/2022 21:56

@Passanotherjaffacake im so glad you had a good outcome. The consultant midwife told me babys can look fine on the scan when they’re not :( she was talking like it’s pretty much a given. I’ve heard of low Papp-a and high bhcg but 0.2 Papp-a results seem to fire out the more serious outcomes from what I’m reading. I am still clinging onto it being a glitch in the matrix x

The waiting is just so hard. I should hear Thursday . Whilst the NT is ‘normal’ it’s still above the 99th centile so I’m feeling pretty gloomy. I’ll be thinking of you too. Time has never gone so slow

darklight03 · 22/10/2022 22:43

I was also told about the process for
termination making me feel like it was an inevitable outcome 😭

OrangeBlossomsinthesun · 22/10/2022 22:49

DD is 11 now and doesn't have Downs. I had the screening in pg and was given a high risk of Downs because of high Bcg and low Papp A. Her nucal fold was well within normal range, nasal bone I think was all as should be.

I don't remember the numbers but my risk was put at 1 in 68, so not as high as yours but I was told it was categorised as high.
I had an amniocentesis which was negative and the rest of the pg was normal.
I really hope you get the news you are hoping for.

Mmr224 · 23/10/2022 08:45

My papA was around 0.32 and bhcg about 1.89 for 1st baby and 0.4 vs 1.78 for 2nd baby. Both fine. I was in my 40s and both were ivf pregnancies so seen as high risk anyway. Like a previous poster these numbers seem to be normal for me.

KellyMarie84 · 27/10/2022 08:02

@darklight03 i know what you mean. I have been an absolute wreck. Cut myself off from the world, apart from dragging myself into work. Have you heard anything yet? I got a call last night for then to say my results have been delayed and will be a couple more days.

i hope you’re doing ok! X

OP posts:
KellyMarie84 · 27/10/2022 08:07

@Mmr224 thank you for your response. I’m still waiting for my results x

OP posts:
darklight03 · 27/10/2022 08:55

@KellyMarie84 You've done better than me! I haven't even managed to drag myself into work and I literally have had 2 days sick leave in 20 years. I'm still waiting too, hoping to hear something by the end of tomorrow... urgh the waiting is unbearable.

KellyMarie84 · 27/10/2022 10:50

@darklight03 i had the day off when I received my initial “greater than 1 in 5” result- I was due to go into work after my appointment but fuck that, I’d have been no use to anyone and couldn’t stop crying. But I very recently changed jobs and I’m through an agency so i have no employment rights yet really so need to be coming in. But I just want to curl up in a ball (and drink 😭), but obv can’t do that either. Every cramp or twinge I have, I think maybe I’m having a miscarriage n part of me is relieved as then it will all be over, but I know that’s my anxiety. This week has sent me to some very crazy places mentally. Still praying hard for us both! X

darklight03 · 27/10/2022 22:12

My results confirmed T21 :( Hope you are the other 1 in 2

KellyMarie84 · 28/10/2022 08:42

@darklight03 I’m so sorry I’ve been thinking about you constantly. My results were worse than yours and I’m older so I really feel I will be getting the exact same phone call by the end of today. I don’t know how to DM on here but I am sending you huge hugs xxx

OP posts:
darklight03 · 28/10/2022 10:07

I’ve been thinking of you too. Hope you have better news than me today. Hoping I’ve taken the bad one in two, and you get the good one. The waiting is just so horrible

KellyMarie84 · 28/10/2022 10:50

You’re so kind, even in your heartbreak you are still thinking about others. I want the wait over but I’m not ready to accept the news either. Such an awful time xx

❤️

KellyMarie84 · 28/10/2022 16:27

I wanted to update. I received my phone call and unbelievably it has came back low risk. I’m in a state of disbelief and Convinced they have mixed up my results with someone else’s. I’m still trying to digest the emotional rollercoaster that has been the last 9 days and thank you for all your responses.
@darklight03 i hope you are ok. It’s all so unfair xx
