MoanerLeaser thanks. I won't go into gory details but it needs to be done. My local hospital has a pre-op dept but doesn't do pre-ops (you couldn't make it up) so i need ambulance transport to take me 15 miles to a bigger hospital where they use the same forms which currently have three ticks or crosses against each question for each of my three visits and I'm reluctant to make it four without a date for the op. A shocking waste of money.
