Could someone help me with spelling grammar on this form

Can someone help me with spelling and grammar please ? I wouldn't normally ask . But its important as its for ds i don't even know if I have done it right or if I should ask for help . Thank you

As a parent you are an expert on your child. Whether you are requesting an Education, Health, and Care (EHC) needs assessment, or preparing your notes for an important meeting, it is important to give clear information about your child's needs.

About your child’s needs now

Tell us your views
Health - eating, sleeping, illnesses, tiredness, depression, panic attacks.:

Max eats ok but gets stressed over trying new things. Smells. 

Physical skills - walking, climbing, handwriting, using scissors.

Max writes younger than his age

Communication - hearing, gestures, eye contact, speech: describing things, talking to people, using the telephone, taking messages.

Max can physically hear. But doesn’t take things in well. Example if I say get your shoes from under your bed. And a jumper. By the time he gets to his room . Hes forgotten.

Max can make eye contact but it can be hit and miss . It depends on who it is. And how max feels ie if he comfortable with the person if he is nervous or scared .

Max speech is ok although there are some words he can’t prounce. He can loose track of what hes trying to say and be come muddled. He can miss out key information.

Max can describe basic things. But not more complicated things. Such as i asked him to tell me the way home. Using his hands he said you go this way then you stop and turn that way then you walk some more etc . But he was not describing landmarks how many roads to cross before turning.nanes of roads etc .

Max talks to people at home to family . But will close down if he becomes over whelmed or the house is to busy for him. He can get frustrated if he doesn’t feel listend to. Or if someone over talks him he will forget everything he was trying to say.

He can say words but often finds it hard to put them together to say what he wants to.

Max finds it hard to use the phone . He stutters and gets confused. Uses short one word answers. Or gets mixed up with order of words. Hes not sure when to speak.

Max will not remember a message so it will not get passed on. The most he can do is say mum wants you to his brother . That’s if no one else has distracted him in anyway.

Personal skills - dressing, washing, dealing with pocket money, timekeeping, remembering to pack sports kit/or pencil case.:
Max needs prompting when dressing for school. He wont notice thing’s like a dirty clothing. He needs prompting with washing.

He does not understand pocket money. Example if he has 2.00 he will look at things that are 5.00 or if hes spend 1.00 he thinks he can get lots of things.

He has no concept of time.

Max never remembers pe/swimming kit.

Your views on her behaviour i.e., showing anxiety or frustration, problems with concentration

Max will express anxiety on the way home from school in different ways sometimes it will all spill out other times he will want to walk on his own and be alone. And not want to talk.

And will become withdrawn.

Max cam not concentrate for very long as he forgets what he should be doing.
Your child at home i.e., Watching TV, reading, hobbies
Max likes to watch YouTube. Play on will and time on Xbox.
Outside activities – any clubs, sports.

No clubs. He does enjoy going to the park riding a bike and roller skates .

Her relationships - parents, brothers and sisters, other adults, friendships:

He has a good relationship with family. He does like to be on his own as well. Max does not have friends outside of school. Max is quite around other adults 

Behaviour at home – how is her Sharing, listening, fighting with siblings, moods, any tantrums?

Max can become frustrated and argue with siblings . He will  say things like I hate you. Get upset shout etc. Then give up and become withdrawn .

Homework – any difficulty remembering what to do or finishing in the set time.

Max does not do home work.  I have never forced this as max struggles with the school  day . It takes  alot out of him during the school day as he tries so hard and keeps things bottled up and he worries alot. Max need his down time  where he doesn’t  need to worry about school work.

What lessons or activities does your child enjoy?

Max enjoys art  topic work .

Your view on her friendships/relationships with teachers

Max had 1 friend he gets on well with.

Max gets on well with teachers that he feels listend to and have a gentle /soft tone.

He gets scared when teachers project their voice. Or shout.if hes told things like its not good enough he will hold on to that for a long time and it will damage his confidence.

Problem areas: during lessons, playtime, new teacher, change in routine

I don’t really get told anything from school. I know that max does not do computers due to feeling scared of the teacher I think due to projecting of voice.

It would depend on the teacher and who the TA is

Extra help or support which has worked,

or has not worked for your child.:

Max has a TA who’s not a true one to one . But spends alot of time with max sitting with him and he has alot of help. Being patient and gentle the help he gets from the TA is fantastic.

In year 4 max did not get the help he should have. Max became withdrawn scared of going to school. He was told things like this is not good enough. And shouted at.

Are your child's difficulties getting worse?

At present no . But I do feel they will once he gos to secondary school.

Does any incident or piece of work illustrate your child's difficulties?

Yes  looking at max books he writes younger than his age   most work is not independent.

What help do you think your child needs?

I do believe max needs are being met at present.

However I believe this will change in secondary. My understanding is max will not have a TA that will be able to spend alot of time with him. He will become confused with having different teachers who have different approaches.

Max can not hold in information very well he will forget whats said to him very quickly and forget instructions. If hes told how to do something he will not say he does not understand. And will sit waiting without letting anyone know. If he is not comfortable with the teacher/person helping he will become withdrawn and not talk.

If max is shown how to do something where he has to xyz he will only be able to do x . He will forget y and z . Once they have been done after having help he will then forget all of the xyz again.

Max can not read instructions. As his reading level is roughly year 1. And he has to sound out most of the words.

I filled in a form called parents views which i put into this op.

I thought it meant that to but wasn't 100%

Thank you . She rang me. And said if i fill in the template. She will go over it . But now I cant find the template. Im now wondering if thete was one . Im not sure if I have confused myself cant vanish if its an attachment can it 😭

I sent of the firm via email on the 22nd August. I asked if she could ket me know if she got it and what the next steps are. But there was no reply. I sent another email yesterday 4th August. Again no reply. Am I wrong impatient?

Im meant to get ds school choice in by 19th September im not sure how. If I don't know what's going on.

If i don't hear anything shall just go to the next walk in session ?

Unfortunately, it’s likely due to the summer holidays. Anything to do with education seems to shut down until 2nd September.

That's not been done. I can probably try and do it myself. But I don't know who in the LA im sending it to . And im not even sure if I still have the support/help from SENDIAS because im not getting any replies.

All i can think is to go to the next walk in which is mid August. But I don't know if im being impatient

  • When trying new things, what do you see Max do? Does he refuse to eat, shout, tears, scream, throw the plate?

Max writes younger than his age.

  • What do the teachers say Max can do and what is the difference between that and where he would be expected to be?

Max can talk to people but will shut down if the house is too busy.

  • What does Max do? Does he leave the room, does he become mute, does he cling to mum with his head in your neck?

Outside activities

  • Max does not like social activities. He will cycle or walk in the park in his own.

I hope this helps with clarity.
