I am handing in my notice...need to write the letter

hello. Writing my notice letter as I amnot going back after maternity leave.

What do I need to include?
Who to I address it to? ie if I saw my boss I would call him by his first name, but is it ok to put this in the letter? Or do I need to put Mr X? Its a school so he is the head?

How long does it need to be?

If you would normally call your boss Fred, it's fine to address it Dear Fred.

You could put something like

'After careful consideration I have decided that I will not be returning to work after maternity leave. Please therefore take this as my one month's notice (or however long your notice period is). I would like to take this opportunity to thank you for all your support during my time at Acacia Road Primary School, and to wish you and the school all the best for the future.

You could ask for confirmation in writing of when your last date of employment will be, and information about say, pension arrangements or anything else that you think might be important.

Thank you flowery, I have just written it and remembered I have no paper!Going to have to find somewhere to print in the morning as U have the meeting at 3.30 tomorrow!
