Over two years I’ve had 56 days off, with 49 of those being since Feb and now extended to May.

This has been because of work related stress and family stress impacting on a disability I have. Plus the physicality of this job has been impacting constantly on pain levels.

From what I’ve read you will have a discussion with occupational health and I would obviously be honest with them and the steps I’ve been doing to be well again, one of which is moving to a much less physical role which this is.

However I keep thinking I’m just going to be rejected.

Kindly - You’ll never know unless you speak to them. You can self refer instead of waiting for a management referral.

I’d imagine you could ask if there is a possibility of you being redeployed to a less physical role. Your condition may apply under the Equality Act. Are you in a union? They may be able to offer extra support and accompany you to your return to work interview if required.
