Letter of intention for mat leave?

Does anyone know how to write a letter of intention for maternity leave for their employer in the UK? What do I need to include?

You can google some templates. I’ve just had one from one of my team, all it legally needs to say is that you are pregnant and what day you would like to take your leave from, and include your mat1b. You should also include any linked requests for annual leave, eg “I would like to take a weeks annual leave from 22 January and then start my maternity leave on 1 feb”.

helpful things to include (for you) would be to set out your understanding of the maternity pay you will get, whether you can keep work equipment (laptop, phone, car ect), and ask about KIT days, work social events, and any work specific things you want to clarify.

helpful things to include for your employer would be how long you intend to be off for (it’s fine to caveat this that you may change your mind with 8 weeks notice in line with your statutory rights, but it’s really helpful to have an idea of intent), and how and when you would prefer to be contacted during maternity.
