Chronic illness early on in new role - best options for managing?

I’m almost 6 months into a new role, fairly entry level in professional services. I’ve career changed after working in education and part time around the kids.
It’s full time in-person with a 2 hour round commute, which in itself has been a struggle due to DH working long hours (and weekends!) in his business- we are really stretched for childcare.

Anyway I’ve developed a chronic condition and had 5 days off so far. I was in A&E last night but went back in to work today after being discharged - I need more tests but it’s not critical thankfully. I couldn’t face calling in sick again!

I don’t know what to do. My boss has indicated before they wouldn’t approve wfh or part time when I asked but I could still submit a request. (A lot of the company are hybrid but not our team.)

I guess I just want to be well but maybe am in denial that I need a month off to really get sorted/condition under control.

I am just scared if I lose this role (still in probation on one week’s notice) it will be so hard to get back into work again.

Just seeing if anyone had advice or experience applying for jobs when you left one due to ill health? Or the best way to manage this with my employer?
