Enhanced DBS

Hi

I'm hoping for some advice please.

i have an enhanced DBS on the update service.
recently i started a new job and although they checked the update service and said everything was fine it was their process to do a new DBS. Absolutely understandable and completely fine.
I submitted everything and after 6 weeks it was withdrawn because of a name change. I hadn’t submitted enough information.

My work place have agreed to pay for it to be resubmitted. Which I’ve done. The weird thing is I was born with my mothers surname so I have a birth certificate for that then took on my fathers surname a few years later after they married so I have a birth certificate with that name and as an adult I have married and have taken on my husbands surname.

i have submitted my passport, drivers license and marriage certificate but it never asked for birth certificates. I can only send 3 proofs so I’m confused as to how I prove this?

I don’t want it withdrawn again and risk my job I already have a dbs that’s renewed yearly so I’ve obviously done all this before but not sure what more I can do.

had anyone had anything similar and can offer advice please?

Sounds like you need to use passport, later birth certificate and marriage certificate instead of your driving licence in the 2a category.

Make sure you put an explanation in the all names section;

Section a - your current and previous names
If a field doesn’t apply to you leave it blank. Don’t enter N/A or any variation.
You’ll be asked by the employer to confirm all of the details that you provide in this section.

  • enter your title in section a1
  • enter your current surname in section a2
  • enter your current forenames in section a3
  • indicate whether you have been known by any other names by selecting yes or no at a4
  • if you select yes you must provide all other names you have been known by from birth
  • for each name you provide, you must make sure that the forename and surname fields are both completed
  • you must provide the from and to dates for every name you have been known by using the MMYYYY format only. Make sure your dates are entered in descending order

Thank you for replying.

i did think that too but it said I had to prove my identity so I used passport then prove address with a different document so I used driving license then my marriage certificate shows my maiden and married name but there wasn’t anywhere to add my birth surname and no where to be able to explain.

I sent them an email but I’m guessing they won’t be working weekends and I had to submit it before a certain time.
