How can I improve this situation?

Background to this - my colleague and I work in the same job role in a clinic which is a satellite to our main clinic site. They started there a year before I did, however I had already done the same role in another clinic for two years previously, so as far as length qualified we are the same. Due to them being employed at this clinic for a year longer, they are paid more than me.
The issue is that we are both on full time contracts - the hours are 7.30 to 5.30 , 4 days a week - and they do not work anywhere near their contracted hours.
They turn up at around 9am, and depending on how many clients we actually have booked in they will go home at anytime between 2-4.30 pm, with all sorts of reasons for leaving early (I swear their car has had 3 MOTs this year...)
Essentially they are working part-time hours (approx 20hrs p/w) and being paid full time. I am also having to pick up allll the slack.
I've tried to be so accommodating, and suggested they apply for flexible working and/or speak to our manager to let them know they are struggling to do full time hours. What else can I do??
