I am having some medical procedures done next month for a week in a hospital two hours from home so will stay over. My consultant rang me with the dates and then sent a letter out with what will happen on each day but no dates on the letter.
My manager has said I need to provide him with a letter stating the date of the procedures.
Do I have to provide my manager with a letter, I thought I could self cert for a week.

You are correct, you can self certify for the first 7 days (including weekends) and usually hospitals would require you to self-certify first and only provide a certificate to run from day 8.

Is your manager referring to a policy about providing letters for medical appointments? Maybe he's misunderstood and is thinking about the medical appointment policy rather than sickness absence policy?

Depending if the surgery was elective/cosmetic or medically necessary - in my company you would need to take A/L for cosmetic surgery. Medically necessary would require a letter if you are scheduling in advance - planned absence is different from unscheduled illness or accident.

@BloodyAdultDC Its for medical procedures and the letter I have explains what will be done on each day for 4 days, however it does not state what date just the days.

You could ask your consultant's secretary to send a letter confirming the dates.
