I’ve been offered a great job and need to fill in a screening questionnaire, I’ve had mild mental health problems but nothing that’s ever affected work, do I legally have to disclosure this?

You need to answer the questions truthfully. If you don't and you are then off with a condition you didn't disclose you already suffered from, they could terminate your employment.
But don't volunteer anything they don't ask about.
Mild mental health problems are very common so I wouldn't worry about that at all
