I've recently been accepted for an applications engineer role at my local police station, I've completed all the vetting and medical forms. There is a seperate sickness absence history form which I am confused about, it asks to provide details of absence for the past 2 years.

I actually haven't had any absence in the past 2 years in my previous job, but there isn't an option to write no absences. It only has the option to enter dates of absences, has anybody else dealt with these types of form?

Just leave it blank or write across it that you haven't taken any sick leave.

Just put n/a in every box??

I'm guessing this is an online form that won't allow the field to be left blank?

Could you put one day, say 01/01/2021 - 01/01/2021 and add a note wherever there is a suitable free-format field to say your absence is nil but the form mandated a date entry.

Write 'none' in the first box or diagonally in large letters across all the boxes.

Or will it accept 00/00/00 - 00/00/00?
