Fit note with maiden name issued

Hi,

I got married few months ago and changed my surname with work etc.

I moved into my husband’s place. However during this marriage, I have kept living between his house and my mum’s (where I previously lived). They both live in different boroughs.

I did not change my doctor surgery as I have been with them for 20 years and really find them good. I did not get around to change my surname with them too.

I have recently been signed off work. I have a fit note issued but it has my maiden name on. It also has my mums address on. Would this be okay?

It shouldn't be an issue, but it's really a question you have to ask your employer, as it's them that has to accept the fit note.
