Do managers print off OH reports and put them into staff members confidential files?

We don’t have paper files now days - all IT based so save a pdf copy onto the electronic personal file if that helps.

Same here - but if the question is "do we keep OH reports on file?" then the answer is yes. If the question is "do we print them out and put then in a cabinet?" then answer is that we haven't had secure cabinets for years now! Although the finance and legal people do have storage vaults under the Town Hall.

We do. It's the dark ages where I work and we have paper personnel files in a locked cabinet with only supervisors/ managers having access. All reports/investigations etc are kept there.
