Significant time off with mental health problem

I’ve had a significant time off work over the last couple of years with mental health issues. This has included a lot of single days due to the nature of my problem. I’ve been with my employer for 13 years and they’ve been very sympathetic to date. My GP is very supportive and they’ve asked for a medical report from her. I’ve been told by my employer that if this goes on, that disciplinary measures would need to be taken. Can anyone tell me whether this would come under a normal disciplinary procedure and have to follow the usual process, ie trying to sort things out informally, then a written warning etc? Or can they simply ask me to leave because of the amount of time I’ve had off and that so much of it is in single days?
