I have applied for another job.
My sickness this past year hasn't been great but I've had covid twice and problems that I've been seeing a neurologist for
He thinks it's caused by an issue I have had since 2015.
Does a reference detail the reasons why you were off or do you have to explain that?

Usually the reference will detail how many episodes of sick leave and duration. The cause is not discussed within the reference.
I would be upfront with disclosures during the interview with the appointing manager.

Our references ask number of sickness absences in past 22 months, that's all the info that can be given about it.
If it's thought to be excessive then you would likely be referred to Occ health for full assessment of reasons of absences and to check if it's going to be recurring problem needing more time off sick

The reference request would just ask for number of sick days, not reasons.
