Work will not make reasonable adjustments for me

I cannot go too much into specifics for obvious reasons. I have a chronic health condition which is manageable with certain reasonable adjustments which my workplace could easily accommodate (think working from home part of the week in a very standard office job that doesn't require any client contact, as an example). Work have known about this since April and despite saying they would put adjustments in place have done absolutely nothing. HR passes the buck to my line manager and my line manager passes the buck back to HR. I'm at the end of my tether and have just been signed off for 2 weeks with extreme stress which has in turn caused a major flare up of my illness.

Has anyone got any advice as I feel close to a breakdown over this. I cannot leave, I can't afford it and it's a massively niche role.

I think Employer is hoping you'll just leave.

This is stressful. I've been there. Don't leave without exhausting all relevant procedures.
This makes their actions harder to defend.

i would Request a copy of the risk assessment you mentioned under data protection.
at the same time Furnish hr with a letter showing accommodations you require (and report from consultant endorsing same).
State that you have sought meetings with hr since whatever date but haven't had any response.
State that you are happy for oh to have a copy of those recommendations.
State that you'll be back at work on date in accordance with accommodations as set out.
You look forward to any input they may have.

If you do the above, prepare for a fight.
Your other option is to withdraw all requests but even then they may want rid as they are now on notice.

Make sure you have support from family and friends at this time.
