HR/Legal Gurus - help me resign please!

Hi all
Am going to join the ranks of SAHMs and not return from my maternity leave. Fortunately my firm was sufficiently stingy to pay statutory maternity pay and no more - so no financial consequences to not returning. Any idea where I can find one of the standard letters re:maternity leave for putting in my resignation - have been unable to track down on the DTI web site.

How does this sound, folks:-

Dear Sir
As required by my contract of employment I hereby give you 4 months notice of my intention to leave my position as XXXX in the XXXX Division (I am due to return from maternity leave on 9th January 2005.

I have decided against returning to work after my maternity leave. This was not an easy decision and took a lot of consideration.

I would like to thank all my colleagues in the XXXXXXXXXXx for having me as part of their team.
