Is this for a job interview?! I don’t think it’s really the right profession for that kind of discussion, when mistakes can mean people being hurt etc..

I have never been asked a question like this in an interview, why do you think they will ask that and what would you tell them. They might ask about reflective practice but not mistakes surely.

You may be asked about a situation where things didn't go well, and how you handled it. You are unlikely to be asked about your worst mistake.

You can talk about accountability, transparency and knowing the correct routes/ reporting to go through when things go wrong.
