Letter heading for formal letter to employer

Having spent the last two hours compiling a letter to my boss (and her boss) putting forward a proposal for my promotion, I am actually quite chuffed at what I have produced and reasonably confident that it might have the desired effect.
My dilemma is how to head the letter. I have started with Dear So-and-so and ended it Yours sincerely so it appears quite formal as I believe is necessary.

Do I put my home address at the top RHS and also include work address on the LHS and where do I put the date?

Any advice is very much appreciated Thanks

At secretarial college they used to teach your address on RHS with date 3 lines below it, then recipient's add on LHS. Then 3 lines below that the salutation, and below that the subject heading if you have one.

Anything along those sort of lines should be OK. I should point out that I have not worked for years and everything could have changed (but I doubt it). Look at any letter you receive if you want to check.

Thanks Jux. I was just wondering if it should be different for internal mail within the work environment. Would I really put my home address?

However I'd use a memo style for a promotion proposal rather than a letter.

Ah yes, that's what I was thinking of. Sorry to be so dim but can you elaborate on memo style please?
This is dreadful as I was taught all of this at school (some time ago now) but these days I rarely have occasion to write letters as everything in done by email.
