Any research study that involves participants (human or animal) and is so badly designed that it won't answer the primary outcome is unethical. This trial design will not resolve the question of whether puberty blockers improve mental health in children with gender dysphoria, therefore it is unethical.

Cass report has been discussed again recently and it struck me that the author maybe had fear of what saying 'no trial' would result in.

Having fear for yourself if you speak the truth is what got us here.

Being brave is difficult and for many of us we keep silent because we are afraid.

There is evidence from the thousands abused under previous treatment to research first.
