I might be asking for the impossible here, but can anyone suggest where to look for good example templates/wording for equal opp forms, which ask all the right questions around sex and gender, which respect those who both do and don't believe in gender ideology? I have a client at work who wants us to proof read one for publishing (included with a report) and I wondered if I could suggest some good examples which don't upset anyone, as I think their one might. (Well it does me, and I expect most on this board!).

If its an EO form, the questions should be in line with how data is collected by the Census. (because that's what you compare your data to)

And yes I know the Census FU the gender question, with silly language.

But the principle was / is

Q: Sex registered at birth
Optional, do you have a gender identity.

But it may well be that so many people have now been stonewalled that you might have to have a whole introductory paragraph about what the word sex means, and what gender identity is.

And I suspect your clients wont want that!

Take a look on the Sex Matters website, they have some guidance on data collection there.
