That Urban Dictionary entry is ridiculous... I seriously doubt that crumbling Hobnobs over condoms is a thing (outside the sad imagination of whoever made that UD entry) or that it's what's being referenced by the drag name. Far more likely that it's 'OTT' as in 'over the top'.

Either way, involving children in drag is abusive and degrading, and all kinds of wrong. I know drag 'performers' often use hyper sexualized puns as names, including when reading to children in oh so progressive library events. But in this case I think it's a reach to claim the OTT name refers to a sex act.

Steph's packed lunch has a lot of iffy stuff, but this one was a new low.

Quite likely that Miles and Mum have no idea about the alternative meaning of the name, just as the band Joy Division who changed their name to New Order had no idea that both of these name have Nazi connotations.

But you wonder about how the name will be received by some.

Also recall Mum said Miles was no good at anything at school, which suggests A-Spectrum.

Why does not being good at anything at school suggest being on the autism spectrum?
