So devastated to be bleeding again, please help

3 years ago I had constant and heavy bleeding. Got a coil (and biopsy which was fine). This improved matters but spotting continued and 12 months later the coil self expelled.
Coil number 2 was put in and second biopsy done. Also fine.
everything has been brilliant since till about 10 days ago when I had more bleeding than I’d had for two years, like a ligh/medium period. That stopped - great. Then yesterday I started with heavy bleeding, clots approaching 10p size. Cramps.
ive tried feeling for the threads, not very good at that as we weren’t using it for contraception. I think I could feel them and I’m pretty sure it’s not expelled yet although I suppose it could have.
anyway there’s nothing I can do today, just bleed and bleed. I don’t know why this is happening or what to ask for to help. If the coil is in I shouldn’t be bleeding like this so why am I? If it’s not in what should I do? Seems stupid to put yet another in but what’s the option?
I am just so upset. I don’t think I’m through menopause, no symptoms at all. I’ve had occasional breast tenderness etc so thought a cycle of sorts was still in place. I guess worst case is that despite the coil the endometrium has gone bad on me and is now cancerous. I don’t know how likely that is. I have read bleeding could be due to hormonal surge overcoming the coil. I guess that’s the best scenario. Assuming it’s still in place should I be asking for HRT? And if so what do I need? As the problem is clearly too much of one hormone at least. I have been quite headachey this week which suggests hormones are responsible as that’s been a longstanding feature.

Around 20 percent of women get very heavy peri menopausal bleeding. I'm one of them and have had incessant bleeding. Pills did not work and the only measure that did was ablation surgery to remove the lining entirely. In my case that was caused by too much oestrogen and not enough progesterone; my hormone levels were all over the place.

The mirena is often used but it is not suitable for everybody and can expel itself.

I would ask your GP to refer you to a gynaecologist. Alternatively obtain a referral letter from the GP and pay to go private for an initial consultation. Do not put up with this because it can and does impact on your day to day life.

i certainly can’t put up with it, totally been spiralling since yesterday and very tearful. It’s just so frustrating. I’m 49. I just thought it was sorted, the first coil being expelled I could understand as it went in to a very thick endometrium but the second one has been brilliant so I don’t know why it’s gone wrong, makes my super anxious - which of course is less than ideal with peri levels of cortisol!

Is ablation a GA? God knows how long the waiting list would be.

Yes it is an operation under GA. Gynae waits in this country at present are very long in many areas.

As someone who has gone through this I would pay to see a gynae privately for an initial consultation and in hindsight I should have done this in the first place.

I went to my GP and he sent me for a blood test; when that came back as normal they decided on no further action. I asked for another appointment to discuss this and was told I would need to wait three weeks; I was bleeding both daily and heavily then. After a particularly heavy episode I decided that enough was enough and phoned the local private hospital. They saw me that day and they listened and after some more tests I went onto have ablation surgery.

So I’ve been doing some reading, think this has happened due to oestrogen dominance - suspect I’m not ovulating regularly and also some stress has hit progesterone levels. Meanwhile being overweight keeps my oestrogen up. No significant menopause symptoms other than the bleeding. Maternal history is menopause in mid 40s so we’ll past that already. GP appointment next week so will see what they suggest. But I’m thinking weight loss meds tbh. Need to shift some weight to help at least. Ugh.

Are you on transexamic acid? This was a life saver when I started peri meno bleeding. I had terrible clots and flooding. The TXA sort of stops it and dries it up.

I then has a hysteroscopy D&C and mirena and all was fine. The D&C cleared my endometrium out so the Mirena could do its job.

Had a scan last week and the lining was super thin so all working fine.

I second using Transexemic acid in the short term. I have a polypoid lining (not individual polyps but womb is a bit like corrugagted cardboard) plus fibroids due to oestrogen dominance. In my case my uterus is "too baggy" for a coil and they're still arguing on whether HRT will make my fibroids worse but in the meantime the transexemic acid has made a huge difference to the flooding, it's amazing stuff.

I was prescribed it in 2023 but didn’t feel it made too much difference. Dh wanted me to take some on Saturday but a) couldn’t remember dose and b) I need this to be properly sorted not just alleviated. Really angry with myself today. Maybe if I’d bit the bullet and dropped some weight in 2023-now I wouldn’t be here now. Probably need to think again about TA if flooding gets worse. Bleeding has been heavier today after two lighter days which is a bit shit.

Don't overthink it.

Take two tablets 3x a day, up to three days.

Yes you need to get it sorted but this helps in the meantime.
