A+E very heavy menstrual bleeding

Hi, a couple of days ago I was on my 2nd day of my period. I woke up around 6am and my pad and PJ bottoms were saturated in bright red blood. I wasn't in pain but was really alarmed and felt dizzy. Phoned 111 and they said come in to A+E. Previously, I'd have an ultrasound and transvaginal ultrasound which came back completely clear. The doctor in A+E looked over the results of this again and reinstated that everything looked absolutely fine no cysts/fibroid etc. I had blood tests done at the hospital all came back normal.
I was given tranexamic acid which helped massively and have had a very light period since.

What could of caused this since tests have came back clear? I have suffered from heavy periods for a while but this was the worst its ever been. I feel fine now but dreading the next period (they are very irregular) I'm not sure if this matters but I have a BMI of 19 and eat a healthy diet. Possibly lacking in some vitamins. Has anyone used tranexamic acid as a long term solution? I am going on a girls holiday in a couple of months and I'm terrified this happens again when I am away.

May I ask how old you are now?. I ask only as if you are say early to mid 40s now this may have been heavy bleeding arising from being peri menopausal. Constant rises and falls in both progesterone and estrogen levels led to me having constant and otherwise heavy bleeding. Irregular periods can be caused by hormonal imbalances and can cause lining to build up which can then shed like you describe too.

Did A & E not do a internal ultrasound scan on your last visit?. They should have really sent you onto gynaecology as well.

My blood test that the GP arranged came back "normal" as well but progesterone and estrogen levels were not properly looked at or even measured.

Sorry I know you posted this a few weeks back....

I had the exact same thing in may 2021 Actually ended up with a blood transfusion.

Had all the same tests and never found anything. They said it was suspected endo and I got a mirena coil. Quite a few months off on & off blooding but now not had any bleeding in 9/10 months.

However, I'm now 40 and in suspected peri-menopause. The dr is also saying there is a possibility that they heavy periods coincided with the start of that
