PCOS, post natal probs, perimenopause or something else?

Hi -

I am 40 with 2 DCs, the youngest of whom is almost 3. I had IVF to have both of these as have severe PCOS - I didn’t have periods at all prior to having kids. Had 2 EMCS but apart from that all ok with pregnancy, births. Otherwise in good health, am on the slender side of BMI range and fit etc.

Since having children I have had a number of things happen that I think I need to address - primary symptoms are

  • total loss of sex drive
  • pain during the (infrequent) times I do have sex - feels very dry and as if my vagina is too small
  • totally regular periods - this is a good thing but new for me - I now have a normal period every 28 days on the dot!
  • incredibly moody - I feel almost suicidal for 2 days both at ovulation time and prior to my period. Am generally quite irritable but the low moods are really awful

Generally, I don’t feel like myself. I’ve had a general set of bloods and my ovarian reserve is still high (although has dropped a lot over the last 5 years obv), no obvious deficiencies or issues etc.

Am wondering if this could be perimenopause? Or some kind of weird post natal PCOS-related side effect? I assumed it was to do with breast feeding but it’s coming up to a year since I finished feeding my youngest and I still feel like it. (I would also like to genuinely feel like sex with DH one day again rather than just going through the motions!)

Has anyone had similar? Any sage advice? Thank you!!

(Will end by saying that obviously I will need to see a doctor but wondered if the MN hive mind had any insight!)
